Gentamicin Cures Vertigo, but What Happens to Hearing?
نویسنده
چکیده
Instillation of gentamicin in the middle ear to cure severe Meniere's disease is of considerable value in the control of vertigo. The drug causes diminished endolymph production and, most often, a peripheral vestibular loss. After vestibular rehabilitation, led by a physiotherapist, the ataxia should be resolved as a result of central compensation. Fifteen to twenty percent of the patients experience an added hearing loss in the treated ear, although this percentage is reduced if the treatment is given as a single instillation on 2 consecutive days. Using this strategy, the mean added hearing loss for the treated group is zero. There is no correlation between the pretreatment hearing level and the added hearing loss.

Intratympanic gentamicin for the treatment of Meniere's disease and other forms of peripheral vertigo.
Titration therapy with intratympanic gentamicin offers class A or B control of vertigo in 87% (range, 75%-100%) of patients with unilateral Ménière's disease. The risk of additional hearing loss is about 21% (range,0-37%). Vertigo may recur, however, in nearly one third of patients over time. These recurrences can also be treated by intratympanic gentamicin with a similar risk of hearing loss. ...
متن کاملIntralabyrinthine Vestibular Schwannoma Responsive to Intratympanic Gentamicin Treatment.
Intralabyrinthine schwannoma (ILS) is a rare benign tumor that affects the ends of cochlear and vestibular nerves. In a majority of the cases, it occurs with unilateral progressive sensorineural hearing loss. Less frequent symptoms include tinnitus, imbalance, vertigo, or fullness. The advent of magnetic resonance imaging allows early diagnosis and enables an appropriate therapeutic protocol. T...
متن کاملIntratympanic gentamicin for Ménière's disease or syndrome.
BACKGROUND Ménière's disease is characterised by three major symptoms: vertigo, deafness and tinnitus, which may be accompanied by aural fullness, all of which are discontinuous and variable in intensity. While discontinuous, these symptoms are synchronous. Intratympanic application of gentamicin, an ototoxic aminoglycoside, is a relatively new ablative treatment for vertigo in Ménière's diseas...
متن کاملVestibular nerve section versus intratympanic gentamicin for Meniere's disease.
OBJECTIVES/HYPOTHESIS Vestibular nerve section and transtympanic gentamicin administration are procedures with proven efficacy in the treatment of vertigo associated with Meniere's disease refractory to medical management. Hearing loss is a known complication of each of these procedures; however, there has not been a report of hearing results of both treatments from a single institution. STUD...
متن کاملMénière's disease.
PURPOSE OF REVIEW Ménière's disease is characterized by spontaneous attacks of vertigo, fluctuating sensorineural hearing loss, aural fullness, and tinnitus. The pathologic process involves distortion of the membranous labyrinth with the formation of endolymphatic hydrops. This review describes the pathogenesis and etiology as well as the diagnosis and treatment of Ménière's disease. RECENT F...
